COORDINARE interacts with providers and stakeholders in health and community service systems who, in turn, collectively impact individuals, families and communities in our catchment.
We strengthen linkages between primary care and the broader health system through our partnerships with health care providers, non-profit organisations, residential aged care providers, government, community and cultural organisations, philanthropic organisations and tertiary education.

aged care homes supported to access HealthPathways, palliative care and wound care training.

Healthpathways pages localised this year, bringing the total to more than 1,320 pages localised across our two HealthPathways sites.

targeted dementia consultation workshops conducted in the Illawarra Shoalhaven with aged care home leadership staff, LHD, DOHAC and key educational institutes to address the growing need for improved care transitions and to strengthen local dementia pathways.

In 2023/24 we:

Helped develop SafeSide program to provide suicide prevention training for the alcohol and other drugs sector. This was nominated in 'Communities in Action' category at Suicide Prevention Australia's NSW LiFE Awards.

Developed post-suicide response protocol with Southern NSW Local Health District (SNSWLHD), Standby Support After Suicide and NSW Dept. Education, which was nominated at the SNSWLHD excellence awards.

Worked closely with Illawarra Shoalhaven Suicide Prevention Collaborative and the Illawarra Mercury to implement the 2023Care to QPR campaign

Partnered with the SNSWLHD and Illawarra Shoalhaven Local Health District (ISLHD) to support the LinkMyCare chronic obstructive pulmonary disease program, to be implemented late 2024.

Agreed on the priorities of joint regional planning, shared workforce, data sharing and research, and models of care as part of a Strategic Alliance between COORDINARE and the two LHDs.

Worked in partnership with other PHNs to develop a Residential Aged Care Home Telehealth Training Program.

Developed strong relationships with aged care homes, and supported uptake and planning around out of hours preparedness.

Supported aged care homes to access My Health Record and the Australian Immunisation Register, which has resulted in improved data being reported to DoHAC.

Provided Virtual Care Grants to 68% of the Residential Aged Care Facilities (RACFs) in our region, and supported our most regional and remote RACFs to access providers such as geriatrician, general practitioners and allied health virtually.

Surveyed Illawarra Shoalhaven GPs and LHD clinicians on the value of HealthPathways. >41% of respondents use HealthPathways daily. 80% agreed that it's a valuable tool to communicate clinical and referral information.

Evaluated Dementia Pathways with providers in ACT and Southern NSW, finding HealthPathways has an important role in dementia care, providing referral and service information, and supporting GPs with diagnosis and management.

Greater Choices for At Home Palliative Care measure

COORDINARE worked with community, general practice, Local Health Districts, and service providers on the Greater Choices for At Home Palliative Care measure.

This aims to enhance workforce capacity, improve coordination between health services, increase consumer awareness, and enable informed choices regarding preferred places of care and death.

Locals connect to prevent suicide

Illawarra Shoalhaven Suicide Prevention Collaborative's annual Connecting Together event brought together Collaborative members, local stakeholders and community members to learn about and celebrate the Collaborative’s work to reduce suicide in the region.

The event featured lived experience speakers and was an excellent opportunity to network, connect and hear about the ongoing role we can all play in suicide prevention.
